MEDICAL RECORDS
1. How do I obtain copies of my medical records and/or xrays?
In order to obtain copies of your medical records or xrays please complete an Authorization to Release Information Form (see online forms or pick up at our office) and return it to our office. You can do this in person, or by mail, email or fax. The Authorization to Release Information Form is valid for six months from the date signed. We will only release records generated from our office. Medical records from other providers will need to be obtained directly from that provider. Medical records are purged after 10 years from the last date of service.
2. Is there a charge for obtaining my medical records?
Yes, in most circumstances. The fee for copies of medical records are based upon the request and the number of pages requested. Copies of xrays are available on CD for $5.00. You will be provided with an invoice at the time of your request. If we refer you to an additional provider for continuation of care, we will provide your medical records to them at no fee.
3. Is there a charge for forms that need to be filled out by a provider?
Yes, there is a $10 per form fee that must be paid in advance of all forms being completed and available for pick up or sent in the mail.
4. How long will it take to get copies of my records or to have forms filled out by the provider?
Please allow 10 days to complete records requests or form completion.
